I heard they originally wanted to call him Kane The Undertaker but decided to drop the Kane part before his debut. Just like how they also originally wanted to call Mankind, Mankind The Mutilator but dropped the The Mutilator part before debuting him.
At Survivor Series 1990, when he debuted, they just called him “The Undertaker.” However, for maybe 2 or 3 matches after that, they did call him “Kane the Undertaker.” After a couple of matches, they just dropped the Kane part.
i was at SS 1990 and he was introduced as The Undertaker….for about 2 weeks after that he was called Kane the Undertaker and then they dropped Kane and he was forever known as the Undertaker after that…
nope he made his debut as the undertaker with a half mask then he went to kane the undertaker then dropped it as well as the mask

But Kane was dropped from his name a few weeks later.
